Integumentary Physiotherapy Clinic operates a structured continuum of care — from intensive specialist-led rehabilitation through guided recovery and long-term maintenance. Every phase is clinically supervised and designed around your diagnosis.

Continuum of Care

Three Phases of Recovery

Each patient enters the continuum based on clinical need. Progression through phases is determined by your specialist — not by a schedule.

Phase One

Intensive One-on-One Rehabilitation

Direct specialist care for patients with lymphedema, oncology-related functional decline, wound recovery, post-surgical swelling, tissue restriction, and other complex integumentary conditions. Every session is one-on-one with a certified specialist.

Phase Two

Guided Recovery Progression

For patients who qualify for supervised therapeutic progression following the intensive rehabilitation phase. Progression criteria are determined clinically — not by time or patient preference. Sessions may be delivered one-on-one or in a small-group format under specialist oversight, supporting a structured transition toward functional independence.

Phase Three

Post-Rehab Continuity Program

For patients who have completed specialty rehabilitation but continue to benefit from guided recovery sessions. This phase supports maintenance, reduces regression risk, and preserves the functional gains achieved during active treatment. Eligibility is determined by clinical assessment — not self-selected.

Each phase of the continuum is designed and overseen by a certified specialist. Patients do not self-direct progression — clinical criteria determine when transitions are appropriate.

This model is grounded in the reality that complex integumentary conditions — lymphedema, post-surgical swelling, wound complications, and oncology recovery — require ongoing clinical attention, not a fixed discharge timeline.

Phase entry determined by diagnosis

Not every patient begins at Phase One. Entry point is based on your clinical presentation and current functional status.

Progression is clinician-directed

Your specialist evaluates readiness for progression. Transitions are based on documented clinical milestones, not time elapsed.

Long-term continuity is built into the model

For chronic conditions like lymphedema, ongoing guided support reduces recurrence, hospitalizations, and functional decline.

Coordinated with your care team

IPC communicates with oncologists, wound care physicians, and surgeons to ensure rehabilitation is integrated into your broader care plan.
